Chronology workflow
Record workflows for medical records
Medical-record review works best when each event stays tied to the document that recorded it. Build the chronology from dated encounters, keep provider statements separate from later inference, and make gaps or conflicting entries visible rather than smoothing them over.
A verification-first process
- 01
Inventory the production
Identify the facility or provider, date range, page or Bates range, and document type. Flag duplicate pages, unreadable scans, and missing ranges before asking for a chronology.
- 02
Normalize dates without erasing uncertainty
Sort by service date when it is stated. Preserve “reported,” “signed,” and “filed” dates separately, and label an uncertain or inferred date rather than selecting one silently.
- 03
Separate observation from characterization
Keep symptoms, findings, diagnoses, treatment, restrictions, and billing in distinct fields. Attribute each statement to the source instead of presenting it as an independent medical conclusion.
- 04
Reconcile the result with the source set
Spot-check every entry and review every claimed gap, inconsistency, total, or causal sequence against the records and any governing expert testimony.
Cite the encounter and the page
- Include the provider or facility, service date, document label, and page or Bates number.
- When a record cites patient history, preserve the attribution: “patient reported,” not “the record proves.”
- Keep a later summary’s description separate from the contemporaneous note it summarizes.
- Do not infer a missing diagnosis, causal relationship, or treatment gap from silence alone.
Human verification checklist
- Every event maps to an actual page or Bates range.
- Service, authored, and signed dates have not been conflated.
- Provider observations, patient reports, and reviewer inferences remain distinguishable.
- Totals and date intervals are recalculated independently.
Templates for this workflow
Browse all templates →AI Medical Chronology Prompt for Lawyers
Build a date-ordered medical chronology from the complete record, with source citations for every encounter and explicit flags for treatment gaps, prior history, causation language, and missing records.
Special Damages & Medical Billing Summary
Itemize the medical specials from the records and bills — provider, dates, treatment, billed, paid, and outstanding — totaled, with relatedness and missing bills flagged.
Demand Letter Facts Section
Draft the facts and damages narrative of a demand from the record itself — every sentence cite-backed, gaps flagged instead of papered over.
Record Interrogation Session Setup
The standing-rules prompt: paste this once at the start of a chat session and every answer after it comes cited, sourced, and honest about gaps.

Run this source workflow inside a matter.
The planned Workbench keeps the source, workflow run, cited draft, and human review state together. No matter upload or processing is available on this public page.
