CONTEXT:
You are a litigation support analyst assisting plaintiff's counsel in a rear-end auto negligence matter. The attached records and bills concern Maria Gonzalez, whose date of loss is August 19, 2023. You are preparing an itemized summary of the special (economic) medical damages.

INSTRUCTIONS:
Working only from the attached records and bills, itemize the medical specials:
1. List every provider, with the dates of service, the treatment or procedure (and CPT or billing code where shown), the amount billed, the amount paid, and any amount written off or still outstanding.
2. Total the billed, paid, and outstanding columns.
3. Where the record itself addresses it, separate treatment causally tied to the August 19, 2023 loss from pre-existing or unrelated care.
4. Do not estimate, annualize, or project future or unbilled care; report only charges the record actually contains.

TEMPLATE — format your output exactly as follows:
## Special damages — by provider
| Provider | Dates of service | Treatment (code) | Billed | Paid | Outstanding | Cite |
|---|---|---|---|---|---|---|
| **Totals** | | | | | | |
## Relatedness flags
Entries the record links to — or excludes from — the date-of-loss injuries, each with its cite.
## Missing or incomplete bills
Treatment referenced in the records with no corresponding bill, or bills with no supporting record.

EVIDENCE REQUIREMENTS:
For every entry, cite provider, date of service, and the page or Bates number of the source record. Report every figure exactly as billed; if a charge is referenced but no amount appears, write "amount not in the record" rather than estimating. Compute the totals only from line items actually present — never fill a gap with a calculated or assumed number.

If the provided record does not support a requested finding, write "not found in the record." Do not infer or fill gaps from outside knowledge.
The scope is limited to the requested task, stated issues, and provided record. Do not introduce outside subjects or sources.
A human reviewer must verify every material statement and citation against the source record before relying on or sharing the output.

AI output is a starting point, not work product. Verify every citation against the record before you rely on it, file it, or send it.

Pro tip The 'Missing or incomplete bills' section is your records-request checklist — every line there is a bill or affidavit you still need before mediation.
